New Student Requirements for Entrance into School

Pre-School

1. Proof of up to date immunizations
2. Proof of recent physical exam (within one year)

Kindergarten

1. Proof of up to date immunizations
2. Proof of recent physical exam (within one year)

Transfer Students: If transferring into school from another school, within NJ, the following is required:

1. Proof of up to date immunizations
2. Proof of recent physical exam (within one year)

If transferring into school from another school, outside of NJ, the following is required:

1. Proof of up to date immunizations
2. Medical exam within 30 days of moving into the state of NJ
